  • OPTION A: MONTHLY PAYMENT PLAN

    Annual Registration Fee: This option is similar to an online streaming service subscription where your registration fee is $150.00 due on the first visit, and then every 12 months from the month of the initial visit.


    Individual: $150 is due on the first visit, then $150 per month on an AUTOPAY System.

    **This is in addition to the above registration fee.


    Includes: Unlimited office visits, telemedicine visits (at the provider's discretion), after-hour calls, EKGs, injections including 1mg B12, antibiotics, and steroids, in-house procedures, and no-show charges.


    Excludes: BioTE® pellet insertions, BOTOX®, JUVÉDERM®, skinny shots, IV therapy, and all Menu items mentioned underneath ‘IV and Injection Services’ above.


    Terms:

    • Contract is binding for six months from the date the agreement is signed
    • After six months, you may cancel this contract at any time in writing
    • These fees are likely not reimbursable by your insurance plan
    • Renewals after cancellation will be subject to a new registration fee and the signing of a new agreement
    • Late payments (> 15 days) will be subject to a $150 late fee

    ** This includes declined payments or expired payment methods

  • OPTION B: PAY-AS-YOU-GO PLAN

    Annual Registration Fee: This option is similar to a retail store membership where your registration fee is due every 12 months from the month of the initial visit.


    Individual (> 18 Years Old): $300

    Spouse/Significant Other (> 18 Years Old): $300

    Each Additional Eligible Family Member (Same Permanent Address and < 26 Years Old): $150


    Office Visit Fees (a la Carte):

    • $250: Annual physical visits
    • $200: Lab Follow-Up visits
    • $175: Sick visits and Medication Check-Up visits
    • $200: Telemedicine visits (billed at provider's discretion)
    • $75: After-hours calls
    • $50: EKG and no-shows
    • $25: Per injection (antibiotics, B12, steroids, other)


    Excludes: BioTE® pellet insertions, BOTOX®, JUVÉDERM®, skinny shots, IV therapy, and and all Menu items mentioned underneath ‘IV and Injection Services’ above.


    Terms:

    • All fees are due at the time of service
